OUSD Athletic Clearance

All players for the 2020-2021 School Year, MUST complete a physical, and receive athletic clearance from OUSD Athletic Department.    

• Returning Athletes: Log-in with your user name and email - same process as was used last year.  Complete and turn in your physical form to the office..
• New Athletes: Follow steps on the OUSD Athletic Clearance Process form below. You will create an account and set up your profile.  Complete and turn in your physical form to the office.
• The OUSD Physical Form (available below) is to be completed by your doctor or at an urgent care facility. Return the completed form to the Principal's office. Leave them on the first desk in the paper file tray labeled 'Athletic Physical Forms'.
